Community Participation
Having a disability shouldn’t hold you back from doing what you desire. You can live life to the fullest and get involved in a range of events and activities. SHC ensures the safety and comfort of all our clients are of the utmost importance. We can support you in activities such as:
- participating in recreational and sporting activities
- connecting with social groups or spending time with friends
- visiting your local library
- learning new skills or taking up a hobby or new interest
- volunteering in your community
- participating in camps, classes or holiday activities
- going to the movies or watching a play
SIL – Supported Independent Living
SIL uses a person centered approach where a Participant’s goals, culture, lifestyle and circumstances are taken into account when planning support. The Participant is central to all decision making and all supports can be customized to best suit a Participant’s wants and needs as they change in keeping with the spirit of choice and control as a key aspect of the NDIS.
We account for Tenant Matching Process including but not limited to Participant physical needs, Behavioral presentation, Geographical preference, Professional networks i.e. medical and allied health supports. The range includes but not limited to participants with trauma, relative attachment disorder, physical needs and developmental delay, ASD.
SHC creates designs that meet the present day needs of a variety of participant settings. We offer property management and maintenance alongside meeting and maintaining client needs and is on top of the ever-changing NDIS and residential and tenancies act requirements.
In Home Support/Personal Care
SHC understands your independence is an important part of life. That’s why we offer in-home supports to help participants perform daily activities and achieve their goals.
Daily living assistance supports provided are flexible and personalised to your own needs.
Services provided include help with daily, personal activities such as
- showering, dressing and getting ready for the day,
- mobility and toileting support
- meal preparation and cooking
- domestic chores (e.g. laundry, cleaning, gardening, shopping, etc.)
- assistance with medications
- help with getting to appointments
- companion care
- running errands and general tasks
Nursing and Medical Care
SHC provides complex medical services to NDIS participants . All care is of the highest standard and provided by a registered nurse with experience in the disability sector. We can provide expert care for a full range of services and treatments, including the most complex cases. Some of our services include:
- complex wound management and care
- medications — oral/injections/infusions
- pain management
- tracheostomy management
- diabetes management
- dementia and Alzheimer’s care
- palliative care
- seizure management
Occupational Therapy and Physiotherapy
A team of trained professionals physiotherapists, occupational therapists provide services to the participants of any relevant age. We visit your home or any place suitable for the treatment and comfortable for the participant. Participants can also be taken to the therapist if need be.
Development of Life Skills
SHC helps in fostering individual participant’s skill and helps it to grow and facilitate participants to enjoy the instances utilizing the skill. SHC also helps in development of a participant’s desired skill either individually or through group activities.
SHC staff are trained to design routines where the participant’s skillset is utilized regularly.
